City of Methuen Fire Department - Access to Public Records

* Requests for ambulance run records must be submitted in writing and accompanied by a $5 check and a medical release. The request should contain the patient’s name and date of service. This application with check should be mailed to Methuen Fire, 24 Lowell St. Methuen, MA 01844.

* Request for ambulance bills should be made to New England Medical Billing by calling 1-888-771-6115, Monday - Friday.
