Public Records Request

Note: Requests for ambulance run records must be submitted in writing and accompanied by a $5 check and a medical release. The request should contain the patient's name and date of service. This application with a check should be mailed to Methuen Fire, 24 Lowell Street Methuen, MA 01844.

Note 2: Request for ambulance bills should be made to New England Medical Billing by calling 888-771-6115, Monday through Friday.
